OMNI Youth Services is grateful for your partnership with us as we partner with parents and the community. OMNI respects client rights and client confidentiality.

OMNI is dedicated to partnering with parents and the community to provide innovative transformational behavioral and educational support services to children, adolescents, and young adults that create pathways to successful and healthy behaviors throughout life.

Instances in which your information may be collected include:

  • If you email us or fill out an online form with your email address or make a donation

We may use this information to do the following:

  • Send you program information

  • Enable you to make a donation

  • Update you on volunteer opportunities

OMNI Youth Services will never sell, rent, lease or exchange your personal information with other organizations.

OMNI Youth Services adheres to all privacy and disclosure laws and policy. Information may be shared with your written consent in order to do the following: 

  • Coordinate and/or manage treatment

  • Coordinate and/or obtain payment for services that were provided

  • Document in OMNI’s electronic health record

Information will only be disclosed without your consent only in the case of emergency, when there is a court order, or a few other rare instances. 

OMNI Youth Services utilizes the electronic submission of data to various funding sources, web-based assessment instrument sites, our own electronic health record, email and other internet-based systems. The agency places a high importance on keeping information safe and utilizes available tools in order to insure this. There are risks inherent in these systems, including unauthorized access. OMNI Youth Services minimizes these risks through standard industry standards including the use of user IDs and passwords, equipment that limits the access to authorized users, encryption, and multi-platform security system. Organized Healthcare Arrangement 
OMNI Youth Services participates with other behavioral health services agencies (each a “Participating Covered Entity”) in the IPA Network established by Illinois Health Practice Alliance, LLC (“Company”). Through Company, the Participating Covered Entities have formed one or more organized systems of health care in which the Participating Covered Entities participate in joint quality assurance activities, and/or share financial risk for the delivery of health care with other Participating Covered Entities, and as such qualify to participate in an Organized Health Care Arrangement (“OHCA”).  As defined by the Privacy Rule.  As OHCA participants, all Participating Covered Entities may share the PHI of their services recipients for the Treatment, Payment and Health Care Operations Purposes of all of the OHCA participants.
